Maternity: Pre Admission

Pre Admission

Pre-admission is an important part of your hospital care. To ensure we can confirm your admission, financial and other arrangements, we ask that you:

1. Fill in the online admission form or complete the admission form that you would have received from your doctor as soon as possible and send it back to the hospital.
   
   Please have the following ready when you fill in your admission form:
   1. Personal/Next of Kin details
   2. medicare care
   3. funding details (eg DVA, Private health insurance, workcover or self funding)
   4. Benefit details (eg pharmacy benefit card or pension card)
   5. items numbers if these were quoted by doctor’s rooms
2. If you have any concerns with step 1, please telephone 1800 555 549 between 7am – 5pm Monday to Friday or send an email to preadmissions.nwb@ramsayhealth.com.au

What to bring into hospital...

• Your health insurance details (if applicable)
• Four packets of sanitary pads – 2 maternity / 2 regular
• Two packets of breastfeeding (nursing) pads
• Nightwear and casual wear for day time
• Dressing gown
• Slippers / comfortable shoes
• Bras (nursing bras if breastfeeding)
• Formula (if you plan to bottle feed)
• One going home outfit for baby

Patient Information

If you are a member of a health fund it is important prior to your admission to check with them regarding the following:

1. That your level of Health Fund Cover adequately covers the cost of the procedure and accommodation outlined in the Pre-Admission Form (eg: in the case of post natal patient, is your new born baby covered).
2. If an excess is payable for this admission. This would need to be paid prior to your procedure
3. If you have been a member of your Health Fund for less than 12 months your fund may not accept liability for the costs of this admission.

Maternity Tours every 2nd and 4th Saturday of the month 11am – 1pm.
